The initiative of clinical justification and documentation that interventions to deescalate the patient failed is important to reduce the incidence of seclusion and restraint. Supported care environment that had a high staff to patient ratio, staff training in crisis prevention interventions, talk therapy with agitated patients, and providing a quite area for disturbed or agitated patients had significant impact on reducing the need to use seclusion and or restraints (Vakiparta et al., 2019). The pros of the initiative are to gain control of the situation and maintain safety and stability to the unit environment in a quick and time effective method, post debriefing is performed after this event and reduction in triggers to event are analyzed. The cons is to completely eliminating the policy and procedure and this would negatively impact stakeholders due to the serious danger and unsafe practice on these units could occur. Nurses and stakeholders are not likely to feel safe with the elimination of seclusion of restrain but favor the use of principles of trauma informed care, empathetic reasoning with patients, interprofessional collaboration among the treatment team to provide best treatment plan for the individualized patient care and goals (Gerace & Muir, 2019).
2.Institutional policies are important to an organization. According to Porche (2017), these policies govern the work environment. Institutions develop and implement policies as well as procedures. These ultimately affect the respective constituents of the institution. The hospital food policies are examples of institutional policies. These often guide the type amount and ratios of healthy foods that should be served in hospitals. States have increasingly taken a renewed interest in hospital food, especially in state-run facilities. The intricate link between food and health has necessitated the development of food policies. Ronto et al. (2018) opine that unhealthy foods and drinks have been major contributors to the disease burden in America. Therefore, the hospital food policies are a bid to improve patients’ health outcomes. The catering department is an integral component of wholesome care. Quality and nutritious meals immensely diminish the need to use unnecessary nutritional supplements.
The policies’ stakeholders include the patients who consume the food, nurses, physicians, hospital staff, catering service staff, and nutrition nurses. Some of the hospital policies have provisions that Medicaid managed care contractors provide nutritious meals to hospitalized patients as well as a certain period after discharge. The pros of such policies include their ability to effectively curb health disparities among certain social classes that may struggle to access nutritious meals. Also, it ensures patients do not eat junk food leading to delayed recovery and prolonged hospital stays. However, its disadvantage is the increase in healthcare costs since the health insurance premiums may increase to cater to these changes. Hospitals also bear the implementation costs of the policy. Hence, they might transfer it to patients
